Kaname Madoka egy teljesen átlagos lány. A középiskola második osztályába jár, vannak barátai, szerető családja, mégis kissé motiválatlan. Az élete teljesen felborul, amikor egy cserediák érkezik az osztályába. Különös módon sokat tud róla, és kissé ellenségesen viselkedik vele. A nap vége felé egy ismeretlen hang kéri a segítségét, amit csak ő hall. Nem lenne Mahou Shoujo anime, ha nem indulna a hang keresésére.
